
Mt. Ascutney Hospital and Health Centers Nursing Home Facility is a place where individuals with separate needs come together to live and be cared for with kindness, consideration, and dignity in a community of caring.
The Nursing Home Facility is a 25-bed Medicaid/Medicare-certified nursing home unit that strives to provide restraint-free care and maximum independence. We are proud of our caring atmosphere, which provides personal supervision and activities geared specifically for each resident. We are one of five out of 45 Vermont nursing homes to receive a $25,000 State of Vermont Quality Incentive Award, an honor we received three years in a row.
Short-Term Care: Since we are hospital-based, we can provide superb medical and skilled nursing care and comprehensive rehabilitation services for people recovering from acute illnesses, accidents, strokes, fractures, and surgery, such as joint replacements. We also offer a special needs room to provide short-term hospice or respite care.
Long-Term Care: We offer a home-like caring environment for people who can no longer live at home due to physical or mental changes caused by chronic conditions such as Alzheimer's and Parkinson's disease, Multiple Sclerosis, Stroke, and other physical and mental conditions.
Personal Care: We take pride in the personal care our resident's receive. Meals are provided based upon each residents individual food preference and nutritional needs. In addition to basic nursing care for each resident, personal laundry is done daily by the housekeeping staff, a hairstylist is available for hair care one day a week, and shopping trips are offered to those able to leave the facility.
Accommodations: The nursing home at Mt. Ascutney Hospital and Health Center features both private and semi-private rooms. Each room has its own bathroom.
Social Life: All residents are encouraged to participate daily in a variety of individual and group activities supervised by our Activities Department and the Therapeutic Recreational Specialist. Special programs, such as birthday parties, community outings, and holiday celebrations are also organized. Cooking, gardening, exercise, and art programs are offered. Arrangements can be made for pets to visit.
Family Involvement: Family and friends are encouraged to visit the facility any time and may assist their relative's during activities and meal times. Family members are also invited to attend regularly scheduled team meetings when residents and staff meet to discuss their concerns and make suggestions for new programs. We feel that it is important for family members to actively participate in their relatives care.
The Referral Coordinator is available to answer your questions about the admitting procedure, send you an application, give you information about the Facility and/or arrange for a visit. She/he may refer you directly to the Nursing Home Social Worker/Case Manager for specific questions concerning financial issues.
