Community Care Coordination

The goal of this service is to improve the quality of life and healthcare in our communities and maintain independence for as long as possible for those
living in the Ottauquechee Region.

*Provides a referral service to private independent community caregivers (formerly the HIRS service), as well as for licensed in-home care needs.

*Offers information about respite care, assisted living, nursing homes and ‘aging in place’ initiatives in our communities.

*Links patients to Medicare and Medicaid along with medications, transportation and financial resources.

*Assists in accessing medical and behavioral healthcare.

Our hours of operation are:

  • M, Tu.,W, F: 8 a.m. 5 p.m.
  • Thursday: 8 a.m. 7 p.m.
  • Saturday: 9 a.m. to 12:30 p.m.

After Hours Coverage

The Ottauquechee Health Center is NOT a 24-hour emergency care center. However, arrangements have been made for round-the-clock phone coverage and, in cooperation with the Woodstock Ambulance Service, 24-hour emergency transportation to nearby hospitals.

On Call Services

If you should have a non-emergency medical need after regular clinic hours, we have established an on-call system whereby you can speak to a local provider of healthcare services. Call the main clinic number: (802) 457-3030 and you will be connected to the Provider On Call.
